Please be advised: Anniversary announcements are published for the 25th and 50th anniversaries, and for each five years following the the 50th (e.g., 55th, 60th, 65th...). This anniversary announcement form has been designed to obtain specific information to assure accuracy. When completing the form, please double-check all spellings. Completed forms should be submitted no later than four weeks after the anniversary date in order to be published. Forms received after that period will not be printed.

I will be coming by the office to pick up the photo about two weeks after it appears in the paper.

PHOTO GUIDELINES:

Send us a clear, black and white or color photograph that is no smaller than wallet-size, and no larger than 8" x 10".

Make certain that the names and addresses of the people in the photo are written clearly on the reverse side of the photo.

If you want the photo returned you must include a stamped, self-addressed envelope that is large enough to accomodate the photo.

If you are picking up the photo, it will be available approximately two weeks after it has appeared in the paper at our office, 1 East Main Street Ephrata. Hours are 8 a.m. to 5 p.m., Monday through Friday.
